PLEASE READ ALL INFORMATION BEFORE REGISTERING BELOW:
PHYSICAL EXAMINATIONS: Please check with the Athletic Office if you are not sure if your physical is valid for the Spring 2024 season. This applies to the Asthma Treatment Plan and Anaphylaxis Emergency Care Plan as well. Riverfront parents, please check with the Riverfront School Nurses if their physical is up to date.
- Students registering for Athletics must have a current physical on file before the student is authorized to participate in practice/tryouts.
- If a student's physical is more than a year old prior to the start of the desired season, print out the Pre-Participation Physical Form HERE. Take the form to the physical appointment for the parent and physician to complete. Once the form is completed, bring the original paper copy to the nurse and or Athletic Trainer.
- If you need an Asthma Treatment Plan, or a Food Allergy and Anaphylasix Emergency Care Plan paperwork, it can be downloaded from above where it says Physical Forms.

Final clearance will NOT be granted until both the online registration process and the original paper copy physical forms are received and completed and our school physician signs off.

Your physical is good for one year, and must be valid on the first day of practice. Please see the chart below for the opening day date.

Your physical must be good from one year from the try-out /season start date.
